-    Assessment to determine if & what services are suitable for care of elderly patients & appropriateness of placement

 

Features

-    Assist older patients determine which kind of care will suit patient condition

-    Required prior to access to residential aged care, Community Aged Care Packages (CACPs) or Extended Aged Care at Home (EACH) Packages

-    Team usually based at hospital or community centre

-    Teams typically include doctor, nurse, social worker, physiotherapist, occupational therapist

-    Teams visit in hospital or in residential setting

-    Assessments are free
