Definition

-  Series of ultrasound examinations +/- CTG during pregnancy to detect fetal hypoxia & prevent pregnancy loss

 

Features

-  Must exclude sleep cycle

-  Performed >32/40 in high risk women

-  May be done >24/40 (viability of fetus)

 

Procedure

-  Pelvic US during pregnancy

-  5 parameters with combined score

-  Fetal movements

-  Normal = 1+ episode for 20+ sec in 30min

-  Gross body movements

-  Normal = 2+ discrete body limb movements in 30min

-  +/- Reactive FHR = Non Stress Test (NST) = CTG

-  2+ episodes of accelerations >15bpm for >15sec with fetal movement in 20min

-  Fetal tone

-  Normal = 1+ episode of active extension with reterun to flexion of limb or trunk

-  AFI

-  Normal = 1+ pocket =>2cm

 

Scoring

-  Each scored 0 if abnormal, 2 if normal

-  CTG done only if another parameter is abnormal

-  0-4/10

-  Risk of fetal hypoxia 91-600/1000

-  6/10

-  Score of 0 for AFI risk of hypoxia 89/1000

-  Score of 2 for AFI equivicol test, repeat in 24hrs

-  8/10

-  Score of 0 for AFI risk of hypoxia 89/1000

-  Score of 2 for AFI low risk, 1/1000

-  10/10

-  Score of 2 for AFI low risk, 1/1000
