Deifnition

-  Radioisotope imaging of gall bladder bile excretion

 

Features

-  Biliary scintigraphy producing functional assessment of hepatobiliary bile excretion

-  Nuclear medicine scan using IV DISIDA radiotracer

-  DISIDA is secreted by hepatocytes into bile

-  Time taken varies from 30min to 4hrs

-  Depends on emptying speed

-  IV CCK can be given to contract gall bladder

 

Indication

-  Cholecystitis

-  Acute 94% sens, 65-85% spec

-  Chronic 65% sens, 6% spec

-  Cholestasis, cholelithiasis

-  Biliary atresia

 

Procedure

-  Patient fasted 2hrs +

-  DISIDA given IV & images taken over time w gamma camera

-  CCK given if required

 

Results

-  Normally gall bladder seen in 30min

-  +ve test (indicates pathology): non-visualisation of gall bladder at 60 minutes & uptake in intestine

-  False +ves: TPN, severe liver disease, biliary sphincterotomy
