Febrile Convulsions

Definition

-  Generalized tonic or tonic clonic seizure of < 15 min in a febrile child 6 months – 5 yrs, typically > 39oC

 

Features

-  Most common seizure disorder in childhood

-  2-5% children

-  Increased risk for epilepsy (x2 normal)

-  Simple 70-75%

-  No inderlying CNS infections or neuro abnormalities

-  Complex 20-25%

-  Focal seizure or

-  > 15 min or

-  Multiple seizure in succession

-  Symptomatic 5%+/-

-  Pre-existing neuro abnormality or acute CNS illness

 

Associated S/S

-  Generalised tonic or tonic clonic

-  Commonly caused by viral illness (URTI, gastro..)

-  Pathophysiology unknown

-  Dx of exclusion

-  Family Hx

-  10% risk if sibling effected

 

Ix

-  Upto 18/12 old do septic workup

-  Blood cultures, LP, Urinalysis

 

Mx

-  No evidence for prophylactic anticonvulsants

-  Recurrence rate 30-35%
