Features

-  Obstetric emergency

 

Causes

-  Maternal hypotension

-  Epidural (should recover)

-  APH, intrapartum haemorrhage

-  Cord prolapse

-  Uterine rupture

 

Ix

-  CTG

-  +/- conside FSE

-  +/- FBS

-  VE

-  Assess dilation & station for suitability for vaginal delivery vs instrumental vs LUSCS

-  Exclude cord prolapse

 

Mx

-  Reversible causes

-  Change of maternal position

-  IV fluids

-  Cease Oxytocin +/- tocolysis

-  Based on VE

-  O2

-  3min bradycardia requires medical R/V

-  6min bradycardia should expect recovery from above measures

-  9min bradycardia expediate delivery

-  12min bradycardia baby should be delivered
