Definition

-    In-patient sub-acute care aimed at improving patient function prior to discharge from hospital

 

Features

-    Sub-acute care of chronic or complex conditions associated with aging, cognitive dysfunction, chronic illness or disability

-    Predominately older people with complex, chronic or multiple health conditions requiring treatment & stabilisation, review of further treatment options or service planning

-    Delivered under management of clinician with specialist expertise in geriatrics (ie: Geriatrician)

-    Multidisciplinary team care

 

Procedure

-      Usually requires referral within in-patient setting

-      Management plan should be documented in terms of negotiated goals & time frame

-      Document ongoing assessment of functional ability
