Features

-  Pneumonia contracted from hospital based pathogen (nosocomial infection)

-  Common in elderly or patients immobilised

 

Causes

-  MRSA

-  Drug-resistant Enterobacteriaceae

-  Pseudomonas aueriginosa

-  Acinetobacter species

-  Stenotrophomonas maltophilia

-  Legionella

-  Aspergillus

 

Mx

-  Low Multi Drug Resistance (MDR)

-  Mild

-  Augmentin DF PO BD 5-7/7

-  OR Penicillin hypersen: Cefuroxime 500mg PO BD 5-7/7

-  OR Penicillin allergy: Doxycycline 100mg PO BD 5-7/7

-  Moderate to Severe

-  Ceftriaxone 1g IV Daily

-  OR Benzylpenicillin 1.2g IV 6/24 + Gentamicin 4-6mg/kg

-  OR Penicillin hypersen: Moxifloxacin 400mg PO/IV Daily 7/7

- High Risk MDR

-  Tazosin

-  OR Timentin

-  + if MRSA: Vancomycin

-  + if severe Gentamicin
