Definition

-  Systemic small vessel vasculitis from deposition of IgA immune complexes

-  Leukocytoclastic vasculitis

 

Features

-  Most common vasculitis in children

-  May present w intusussception

 

Dx

-  3+ of (87% sensitive)

-  Palpable purpura

-  Bowel angina (abdo pain)

-  PR Bleed

-  Haematuria

-  Onset <20 yrs

-  No new medications (DDx Drug induced vasculitis)

-  Biopsy

 

Associated S/S

-  Classic triad

-  Nonthrombocytopaenic purpura

-  Lower limbs +/- buttock

-  Palpable

-  Colicky abdo pain

-  Large joint arthritis

-  Renal impairment

-  May resolve or progress to CRF (usually by 3/12)

 

 

Ix

-  Urine Dipstick + MCS

-  Blood: haematuria

-  Protein: renal impairment, CRF

 

Mx

-  Analgesia: NSAIDs

-  +/- Corticosteroids

-  Shortenes duration abdo pain & reduces progression to CRF if Ix show renal impairment on presentation
