Definition

-  Malignancy of lymphocytes mainly confined to lymph nodes

-  B cells, T cells & Natural Killer cells

 

DDx

-  Myeloid Leukaemias (may involve lymph nodes)

 

Features

-  Many clinical overlaps between lymphoma & leukaemia

-  Lymphoid stem cell  (Pre B cell) = Pre B ALL

-  Mature B cells = CLL

-  Plasma cells (B cell) = MM

-  Most lymphomas B cell lineage

 

Types

-  Indolent

-  Nodular

-  Slow growth

-  Improves w Mx but not cured

-  Small cell

-  Aggressive

-  Diffuse pattern

-  Rapid growth

-  30-40% cured if not death in 1-2yrs

-  Increased sensitivity to chemo

-  Large cell

 

WHO Classification

-  Based on S/S, histopathology, genetics & immunophenotyping

-  1. Non Hodgkins Lymphoma: B or T cell neoplasia

-  2. Hodgkins Lymphoma (Reed Sternburg clonal B cells): classic or nodular

-  3. Plasma Cell Disease

-  4. Immunodeficiency Associated Lymphoproliferative Disease

 

Dx

-  Surgical node biopsy (as much tissue as possible)

-  FNA not sufficient
