Meconium Stained Liquor (MSL)

Definition

-  Fetal passage of meconium into amniotic fluid (liquor) during or prior to labour/delivery after rupture of membranes

 

Features

-  Effect 15% of births

-  5% infants develop MAS

 

Mx of Labour

-  Continuous CTG

-  Light stained liquor

-  Observe post delivery 2/24+

-  Heavy stained liquor

-  +/- FBS

-  Paeds at birth

 

Mx of Infant

-  Infant to resusiteur at delivery

-  Assess for vigor

-  If vigorous (good tone, active breathing, HR >100): standard resusitation as needed

-  If not vigorous

-   Radiant warmer

-   Avoid stimulation

-   Suction mouth & pharynx (12 or 14 French) under direct vision & either

-   3 0r 3.5 EET intubation with Mec aspirator

-   OR IPPV with Neopuff or Bag & Mask

-  +/- Stomach aspiration after resus

-  Admit to SCN if

-   Meconium below cords

-   Resp distress or O2 requirement

-   Need for CPR or prolonged IPPV

-  For heavery mec stained liquor regular obs 2/24 for 12/24
