Definition

-  Focal skeletal disorder characterised by accelerated bone turnover

 

Features

-  3% ppl > 40 yrs

-  Increased incidece: anglo-european

 

Assocated S/S

-  Osteoporosis

-  Hearing loss

-  Bony overgrowth: prominent forehead/glabella

-  Commonly effects pelvis, spine, skull & long bones

-  Bone pain, deformities or fractures

-  Bone tumours (< 1%)

-  Neurological symptoms

-  Cardiac disease (rare)

 

Ix

-  LFT: ALP: elevation

-  X-ray

-  Areas of osteopaenia & osteosclerosis

-  Vertebral: pathological fracture

 

X-ray

 

-  Bone Scan

-  BMD

 

Mx

-  Analgesia

-  Bisphosphanates

-  IV pamidronate 60mg over 4 hours

-  Every 12-18/12

-  Monitor ALP for response
