Definition

-    Goal oriented, time limited, therapy focused care to help older people at the conclusion of a hospital stay

 

Features

-    Aims to achieve/restore older persons independence & confidence after a hospital stay prior to returning home

-    Joint funding between Commonwealth & States but service provided by States

-    Assists by providing time for long term arrangements

-    Provided in either home-like residential setting or in the community

-    Average duration of care 7 weeks, maximum 12 weeks (exceptional circumstances extended a further 6 weeks)

 

Services

-      Low intensity therapy: physiotherapy, occupational therapy, social work

-      Nursing support
